What is the Health Declaration Booklet?
As part of your registration to become a childminder, Ofsted requires an up-to-date medical declaration from you and your local GP. This form involves declaring any current or previous medical conditions, medication, or any medical problems, hospital appointments, or treatments you’ve had in the last five years.
What Sections do I Need to Complete in the Health Declaration Booklet?
Ofsted emphasizes that the form should accurately reflect your current and previous medical status. Ensure all requested information is complete. Your registration will not be automatically refused due to current or past health issues.
For childminders registering on the Early Years Register, you will need to:
- Fill out Section A.
- Complete Section B1.
- Sign the declaration on the specified page.

What Does my Doctor Need to do?
The process is straightforward and typically doesn’t require a special doctor’s appointment. Fill out the relevant sections, print them, and have your GP complete section C based on your medical history.
Remember, the section signed by your doctor is valid for six months.
How Much Will the Health Declaration Booklet Cost?
Costs for the Health Declaration Booklet vary. While some GP surgeries offer this service for free, others may charge a fee.
Where and When do I Send my Health Declaration Booklet?
Once you and your GP have filled out the relevant sections you can send it to enquiries@ofsted.gov.uk with your Unique Reference Number (URN). Send it as soon as you can to Ofsted, you don’t want to delay your registration! It can be sent alongside your registration request.
If Ofsted requires further information they will get in touch with you, otherwise, the form will be accepted on your registration portal.
